Volunteer Clearance Process Instructions
Print, read, complete, and sign all three forms. Completed forms must be taken to the school for which you will be volunteering. Photo identification will be verified by a school employee.
1) Washington State Patrol Identification and Criminal History Background
Complete sections C and D and sign (fingerprint and social security number are not required).

2) Volunteer Disclosure Statement
Mark "Yes" or "No" to each of the questions (two pages).
Sign and date the form.

3) Volunteer Expectation Agreement
Read, sign, and add your phone number.
