Volunteer Clearance Process Instructions Print, read, complete, and sign all three forms. Completed forms must be taken to any school or district office. Photo identification will be verified by a school employee.
1) Washington State Patrol Identification and Criminal History Background
Complete the applicant information section. Sign and date the form.
2) Volunteer Disclosure Statement
Mark "Yes" or "No" to each of the questions (two pages).
Sign and date the form.
3) Volunteer Expectation Agreement
Read, sign, and add your phone number.
